City of Bradford Metropolitan District Council is recruiting a full-time Rough Sleeper Recovery Coordinator as part of the New Vision Bradford partnership. The role requires travel across the district, a full driving licence, and access to a vehicle for work purposes.

You will act as the main care coordinator, delivering assessments, recovery planning, and tailored support to service users through their treatment journey. DBS checks are required, with a flexible approach to working hours.

Qualifikationen

• 2 years’ full-time experience working with people with alcohol and/or drug problems or those with complex needs within the last 4 years.
• Experience delivering case management and care coordination in drug/alcohol or related fields.
• Clear commitment to supporting others to make positive change.

Aufgaben

• Coordinate tailored packages of care for a group of service users.
• Conduct comprehensive assessments and recovery planning.
• Provide one-to-one key work and psychosocial interventions.
• Deliver harm minimisation and support across treatment and recovery journeys.
• Lead outreach to connect with rough sleepers or at risk individuals across the district.
