Job summary

The Methodist Church in Britain is seeking a Digital Health Support role within the East Berkshire Primary Care digital health service. The post holder will monitor a caseload remotely, respond to abnormal observations via Luscii, escalate deterioration to clinicians and support case management for patients with short and long-term conditions.

This seven‑day, 08:00–18:00 role involves working with Integrated Care System partners and nursing or residential care colleagues to promote health,

Qualifications

• Remote clinical services experience and ability to monitor patients remotely.
• Experience delivering services with multi-disciplinary teams.
• Ability to interpret vital signs and assess clinical risk.
• Experience with virtual technologies for case management.

Responsibilities

• Monitor patients on caseload remotely and respond to markers of declining health.
• Escalate concerns to Digital Health Clinicians for urgent contact.
• Assist in clinical case management using virtual technologies.
• Respond to abnormal observations submitted via Luscii platform.
• Work on shared caseloads with Integrated Care System partners.
• Collaborate with nursing and residential care partners to improve health and reduce admissions.
• Assist clinicians with complex patients and elevate clinical data.
• Contribute to onboarding and discharging patients on the patient management platform.
• Deliver health promotion advice under supervision, such as self-help signposting.
• Maintain legible, dated and signed records per EBPC policy.
• Manage referrals for patients and carers under supervision.
