North Hampshire Urgent Care is seeking a Remote Monitoring Clinical Support Worker to deliver proactive, home-based care via digital tools. The role operates 7 days a week from 08:00 to 20:00 from our Farnborough Head Office, with a mix of weekday, weekend and bank holiday shifts.

As a member of the Remote Monitoring Team, you will support patients through regular calls and digital contact, review monitoring data, and escalate concerns to clinicians when needed.

Qualificações

• NVQ Level 3 in Health and Social Care or equivalent.
• Care Certificate or willingness to complete.
• GCSEs (minimum 5) or significant equivalent experience.

Responsabilidades

• Monitor patient data from a digital remote monitoring system.
• Maintain regular telephone and digital contact with patients.
• Assist with onboarding patients onto the digital platform.
• Escalate concerns promptly using agreed protocols.
• Provide basic health education and self-management support.
• Maintain accurate electronic records and ensure data quality.
• Respond to amber alerts and missed submissions promptly.
