Job summary

NHS Jobs is seeking a Remote Monitoring Clinical Support Worker to join the North Hampshire Urgent Care Remote Monitoring Service. The role supports proactive patient monitoring, reviews data, and maintains regular contact with service users to promote wellbeing and early identification of deterioration.

Qualifications

• Role supports monitoring of patients via a digital remote monitoring platform and regular caller contact.
• Maintain regular telephone and digital contact with patients to support engagement and wellbeing.
• Identify deviations from normal parameters and escalate concerns using agreed pathways.
• Provide basic health education and self-management support within agreed protocols.
• Work with the team to ensure accurate records and confidentiality in line with Caldicott guidelines.

Responsibilities

• Monitor patient data from the digital platform, including questionnaire responses and observations.
• Support onboarding of patients onto the digital platform.
• Respond to Amber alerts and escalate as required.
• Maintain communications with patients and carers to promote engagement and safety.
• Work with Clinical Head of Service and RM team to ensure patient care continuity.
