Job summary

Norfolk County Council is seeking a Supported Assistant Practitioner to join our friendly, multi-disciplinary team in North Walsham. The role focuses on assessments, service commissioning and reassessment for adults in the community, using video, telephone or in-person contacts.

You will develop through our focused training programme with supervision and peer support. You will need to demonstrate excellent communication, record-keeping and IT skills, and be committed to delivering high-quality

Qualifications

• Sound knowledge of Care Act (2014) and Mental Capacity Act (2005).
• Experience in safeguarding and working with vulnerable people.
• Excellent communication, record-keeping and IT skills.

Responsibilities

• Undertake assessments, service commissioning and reassessment via video/telephone and/or face to face.
• Contribute to planning and delivering social care support for adults in the community.
• Support the team with documentation, eligibility criteria and customer service.
