Norfolk County Council invites applications for an Assistant Practitioner in the Adult Mental Health Social Care Duty team at Hellesdon Hospital, Norwich.

You will deliver Community-Led Support and relational practice, build meaningful relationships, and support independence and social inclusion, with flexible hybrid working and a 37-hour week.

The role supports recovery-focused care and works within Norwich locality to meet local demand.

Responsabilidades

• Build trusting and effective relationships with individuals experiencing mental health difficulties.
• Engage with people in their communities, homes and local settings, taking a proactive and flexible approach to support.
• Work alongside individuals to understand "what matters" to them rather than focusing solely on presenting problems or service-led solutions.
• Promote social recovery through strengthening connections with family, friends, peer networks and community resources.
• Support individuals to access community groups, activities, volunteering, education, employment and other opportunities that enhance wellbeing and social inclusion.
• Utilise asset-based approaches that recognise people's strengths, abilities and aspirations.
