This tried and tested method focuses on strengthening our front door and encouraging social care professionals to have meaningful conversations with individuals to create better outcomes. It moves away from process-driven 'hand-offs' and promotes connecting with local networks and community support. By fostering these genuine interactions, we aim to enhance the quality of care and support available in our neighbourhoods and communities.

By fostering strong relationships and empowering individuals, we aim to create a collaborative environment where everyone can thrive. To build meaningful relationships with the people we support, this role requires regular in-person work in both the office and community.
SCCE current works in a hybrid manner (office and remote working). Note that this is open for review as we expect flexibility and adaptably to the demands of the service.
