Job summary

Medacs Healthcare is seeking an All Age Continuing Care (AACC) Broker Administrator to join the Sefton and Liverpool AACC Team, supporting the NHS Cheshire and Merseyside Integrated Care Board. This remote, full-time role focuses on sourcing and coordinating care packages for individuals with complex health needs.

Qualifications

• Experience in brokering, commissioning or coordinating care packages.
• Experience working within health or social care services.
• Excellent organisational and administrative skills.
• Strong communication and stakeholder management abilities.
• Right to Work in the UK.

Responsibilities

• Source and broker packages of care for individuals eligible for Continuing Healthcare and related services.
• Liaise with care providers to identify suitable placements and care arrangements.
• Support hospital discharge pathways by securing timely care provisions.
• Negotiate rates and ensure value for money when commissioning services.
• Maintain accurate records and documentation relating to care packages.
• Work collaboratively with NHS teams, local authorities, care providers and families.
• Monitor provider availability and capacity.
• Support reporting, auditing and financial processes within the service.
• Escalate concerns, risks or service issues where appropriate.
