Medacs Healthcare is recruiting an All Age Continuing Care (AACC) Broker Administrator to support Cheshire and Merseyside ICB in sourcing and coordinating care packages for individuals with complex health needs, working remotely. You will liaise with NHS teams, care providers and families to arrange timely, cost-effective Continuing Healthcare services and hospital discharge pathways.

Strong organisational and communication skills are essential.

Qualifications

• Experience brokering, commissioning or coordinating care packages for NHS or social care clients.
• Experience in health or social care sectors is essential.
• Strong organisational and administrative abilities with attention to detail.
• Excellent communication and stakeholder management in multi-agency settings.
• Ability to manage multiple tasks and meet tight deadlines.
• Experience maintaining accurate records and using IT systems.
• Proven negotiation and value-for-money skills.
• Must have Right to Work in the UK.

Responsabilités

• Source and broker care packages for Continuing Healthcare patients.
• Liaise with care providers to identify suitable placements.
• Support hospital discharge pathways with timely care.
• Negotiate rates and secure value for money.
• Maintain accurate records and documentation.
