Job summary

Peabody Community Foundation is seeking a Relief Support Worker for Young Persons across Hertfordshire. You will provide person-centred support to young people aged 16-25, including assessments, support plans and practical help with daily living.

You will liaise with partners, travel between schemes and maintain clear records, all while safeguarding and promoting equality. The role focuses on empowering young people to build confidence, access education and housing options, and thrive within

Qualifications

• Experience supporting vulnerable people in the community or a supported living environment, either professionally or through volunteering.
• Strong communication skills and the ability to build positive relationships with young people and external partners
• A compassionate, empathetic approach with a genuine desire to make a difference in people\'s lives
• Confidence to work independently (including lone working) and collaboratively as part of a team
• Good IT literacy, including the use of Microsoft Office and data entry systems
• A driving licence and your own vehicle, as this role will involve some travelling between schemes.

Responsibilities

• Provide high-quality, person-centred support to young people in supported housing.
• Conduct needs and risk assessments, develop and review support plans.
• Assist with daily living skills such as cooking, cleaning, shopping and finances.
• Help individuals build confidence and life skills to achieve goals.
• Support access to community resources and sustain housing.
• Liaise with external agencies, attend team meetings and training, maintain records.
