Responsibilities

• Support and empower individuals to develop or improve independent living skills as identified in person-centred plans.
• Work closely with Team Leaders and managers to prepare and implement individual and group support programmes.
• Collaborate with other Support Workers and Team Leaders to promote a safe working and living environment, resolving conflicts using appropriate techniques and language.
• Ensure that medication is stored and administered in accordance with Praxis Care’s Medication Policy where necessary.
• Familiarise yourself with all administrative requirements of the service and log information according to reporting guidelines.
• Be aware of all policies and procedures, including Health & Safety, and participate in related training.
