Job summary

Bath Mind in Bath is seeking a Relief Community Support Practitioner to provide flexible, person-centred support to adults living in their own homes across BANES. You will join a dedicated community support team and work across client homes, balancing home-based and community visits.

The role focuses on developing recovery pathways, promoting self-care and resilience, and encouraging engagement with wellbeing services.

Responsibilities

• Provide person-centred one-to-one support to jointly agree a personal recovery pathway and support plan with specified goals and interventions.
• Develop strategies for increased self-care and resilience and encourage engagement with other services, such as wellbeing groups.
• Goals are holistic and cover mental health, social and leisure activities, financial management, education and daily living skills.
• Build a therapeutic relationship with the client and assist recovery using resources such as wellness recovery action plans and crisis prevention plans.
