Job summary

Bath and North East Somerset Council is seeking a dedicated Social Care Practitioner to join the Community Pathways Team, supporting adults after hospital discharge to maintain independence.

You will manage your own caseload, complete holistic assessments, and develop person-centred plans while coordinating with partners and safeguarding where needed.

Qualifications

• Experience in Adult Social Care or related health and social care setting.
• Knowledge of Care Act 2014 and Mental Capacity Act 2005.
• Understanding NHS Continuing Healthcare and hospital discharge pathways.
• Experience completing assessments, support planning, reviews and capacity assessments.
• Experience working across agencies and maintaining good records.

About the Team

We are looking for a passionate and motivated full time Social Care Practitioner to join our Community Pathways Team, supporting adults following hospital discharge to achieve the best possible outcomes and maintain their independence.

The Community Pathways Team supports adults who have received reablement services following a hospital discharge and who require ongoing care and support. Our friendly and supportive team includes Social Workers, Social Care Practitioners, Administrative Staff and a Team Manager who are committed to delivering high quality, person-centred services. Working closely with the hospital social care team, health professionals, housing services, care providers and community organisations, we help residents access the right support at the right time.

About the Role

This is a rewarding opportunity to manage your own caseload of adults receiving interim care following discharge home through the reablement service.

You will:

• Complete holistic, strength based Care Act assessments.
• Develop person-centred support plans focused on achieving individual outcomes.
• Undertake carers' assessments and initial reviews.
• Work in partnership with health colleagues, community organisations and support providers.
• Carry out Mental Capacity Assessments where appropriate.
• Contribute to safeguarding enquiries under Section 42 of the Care Act, applying Making Safeguarding Personal principles.
• Maintain accurate case records and ensure timely progression of cases.
• Use professional judgement to effectively prioritise and manage your workload. We are seeking enthusiastic practitioners who are committed to delivering excellent outcomes for adults and their families.

You will have:

• Experience working within Adult Social Care or a related health and social care setting.
• Knowledge and understanding of the Care Act 2014 and Mental Capacity Act 2005.
• Understanding of NHS Continuing Healthcare (CHC) and hospital discharge pathways.
• Experience completing Care Act assessments, support planning, reviews, and Mental Capacity Assessments.
• Experience working collaboratively across agencies and professional disciplines.
• Knowledge of local community resources, support services and funding arrangements.
• Experience using electronic case management systems and maintaining high quality records.
• An understanding of preventative approaches, community based support, assistive technology and strengths-based practice.
• Strong organisational skills with the ability to manage competing priorities effectively.
• Good IT skills and confidence using digital systems.
