Job summary

Bath and North East Somerset Council is seeking a motivated Social Care Practitioner to join the Community Pathways Team, supporting adults after hospital discharge to achieve outcomes and maintain independence.

You will manage your own caseload, complete holistic assessments, develop person-centred plans, undertake carer reviews, and work with health colleagues, housing services and care providers to secure the right support.

Qualifications

• Experience delivering Care Act assessments and developing support plans.
• Knowledge of NHS Continuing Healthcare and discharge pathways.
• Experience working across agencies and maintaining high-quality records.

Responsibilities

• Complete holistic, strength-based Care Act assessments.
• Develop person-centred support plans focused on outcomes.
• Undertake carers' assessments and initial reviews.
• Work with health colleagues, housing services, and providers.
• Carry out Mental Capacity Assessments where appropriate.
• Contribute to safeguarding enquiries under Care Act provisions.
• Maintain accurate case records and ensure timely progression.
• Prioritise and manage workload with professional judgement.
