Job summary

Ceredigion County Council is recruiting a Relief Care and Support worker to join our Team. You will work with service users with complex or high needs and deliver care plans in partnership with professionals to support individual well-being.

The role involves administering prescribed medication, maintaining accurate records, and working across day and night shifts on a rolling rota. You will promote Welsh language and dignity in care, uphold safeguarding, and develop strong relationships with

Responsibilities

• Carry out duties in care and support plans for service users with complex or high needs.
• Deliver direct & virtual 1-1, family or group support in collaboration with other professionals.
• Ensure safe working systems to meet RIDDOR, COSHH, manual handling, first aid and medication requirements.
• Administer prescribed medication including controlled drugs.
• Maintain accurate records and develop relationships with service users and families.
• Identify and manage short- and medium-term risk to keep service users safe.
• Deliver interventions to support mental, social and physical wellbeing and outcomes.
• Work with individuals, families and agencies using a strengths-based sign of safety approach.
• Undertake personal care and use appropriate handling techniques.
• Provide witness statements for legal proceedings when necessary.
• Work a rolling rota of days or nights and cover for training or supervision.
