Job summary

Practice Plus Group’s Health in Justice team in Lewes is seeking a Transfer and Discharge Coordinator to support the Substance Misuse service within HMP Lewes, a category B male prison.

This fixed-term 12-month contract offers Monday to Friday 08:00-16:00 and a £29,000 annual salary. You’ll help coordinate safe patient transfers, reduce delays, and work with community teams to ensure continuity of care.

Qualifications

• Experience with patients within the Criminal Justice System
• Knowledge of local and national policies e.g. NICE guidance
• Knowledge of audit and its relationship to outcomes
• Knowledge of safeguarding adults and children requirements
• Willingness to learn and develop new skills on the job
• Excellent communication skills and clarity in interactions
• Non-judgmental, patient-centred approach

Responsibilities

• Reducing unnecessary and confusing handoffs between healthcare services and inconsistency in service transfer between healthcare providers by initiating early discharge planning or transfer to alternative settings.
• Ensuring safe transfers between establishments which offer continuity of care and ongoing support.
• Ensuring patients goals and needs are considered when care planning prior to transfer, with actions for complex healthcare needs.
• Removing delays for patients accessing and receiving the healthcare and medication they need during the transfer process.
• Working with the recovery team to promote and improve continuity of patient care.
• Meeting with patients to review recovery needs.
• Making links with community drug & alcohol team.
• Ensuring receiving team can meet patient needs (OST prescriptions, appointments)

Salary

This position is full time, you’ll receive an annual salary of £29,000 per annum.

What you'll be doing

As a Transfer and Discharge Coordinator, your responsibilities will vary. You will

• Reducing unnecessary and confusing handoffs between healthcare services and inconsistency in service transfer between healthcare providers by initiating early discharge planning or transfer to alternative settings. This will include transitioning existing care plans.
• Ensuring safe transfers between establishments which offer continuity of care and ongoing support.
• Ensuring patients goals and needs are taking into consideration when care planning prior to transfer, with appropriate actions put in place for patients with complex heath care needs.
• To remove the delay for patients accessing and receiving the healthcare and medication they need during the transfer process.
• Work alongside the recovery team to promote and improve continuity of patient care.
• Meeting with patients to review recovery needs
• Making links with community drug & alcohol team
• Ensure patient's needs are going to be met by the receiving team e.g. OST prescription, appointments in the right place, help link prison and community team
