Job summary

St Peter & St James Hospice in East Sussex is seeking a compassionate Rehabilitation Assistant to join our Therapy Team across the hospice, community and Living Well services. You will support patients to achieve personal rehabilitation goals and provide practical and emotional support to families and carers.

The role involves delivering mobility/ADL programmes, risk assessments, and group activities, with multidisciplinary teamwork and ongoing development opportunities in a specialist

Qualifications

• NVQ Level 3 in Health and Social Care or relevant hospice experience.
• Experience in a hospice, therapy department or care setting.
• Understanding of the role of Physiotherapy and Occupational Therapy in patient care.
• Experience working in a multidisciplinary team.
• Good IT skills including MS Office and electronic patient records.
• Access to own vehicle for home visits.

Responsibilities

• Support the delivery of rehabilitative palliative care programmes under supervision.
• Assist patients to work towards rehabilitation goals, maximising independence and quality of life.
• Deliver delegated rehabilitation interventions (mobility, exercises, ADLs, breathlessness management).
• Support with rehabilitation equipment and aids and home visits.
• Plan and deliver group activities in Living Well Centre.
• Conduct home visits and basic risk assessments.
• Maintain patient records and outcome measures.
• Manage workload including appointments and equipment orders.
• Collaborate with colleagues, volunteers and external professionals.
• Participate in MDT meetings, supervision and training.
