Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking a Rehabilitation Assistant for our Community Therapy Team covering Petersfield and Bordon. You will work with Occupational Therapists, Physiotherapists and other professionals to support patients at home and in community settings, delivering delegated rehabilitation programmes.

You will monitor progress, assist with mobility and daily activities, and ensure patients and carers are engaged in goal setting.

Qualifications

• Previous experience in health, social care or rehabilitation setting desirable.
• A full UK driving licence and access to a vehicle for work purposes is essential.

Responsibilities

• Work under supervision from the Therapy Team Leader, Senior Therapists and Therapists.
• Implement therapy to a designated caseload.
• Assist with clinical/therapeutic activities or treatments.
• Ensure records and activity data are maintained in accordance with professional and Trust standards.
• Prepare patients for and assist with clinical/therapeutic activities or treatments.
• Demonstrate safeguarding knowledge and contribute to safeguarding processes.
• Provide intervention in home and community settings to meet client needs.
