Phyllis Tuckwell Memorial Hospice Ltd is seeking a full-time Rehabilitation Assistant to support high-quality rehabilitative palliative care for patients in the community and at our sites in outpatient and group settings.

Based in Guildford with cross-site working, you will work alongside an Occupational Therapist and other Assistants to empower patients to manage their condition and engage in meaningful activities, improving quality of life.

Qualifications

• A level 3 qualification in Health or Social Care or equivalent.
• Understanding of palliative care.
• Commitment to person-centred, high-quality rehabilitative care.
• Experience in multi-disciplinary team working.
• Strong communication skills.
• Good IT skills across platforms.
• Self-aware and resilient.
• Ability to work with groups of patients and carers.

Responsabilités

• Work as part of a multidisciplinary team to deliver rehabilitation.
• Provide community-based rehabilitation support in patients' homes.
• Assist with outpatient and group sessions.
• Promote quality of life aligned with patient preferences.
