Responsibilities

• Managing medication: administer, order, receive and audit medications; ensure accurate recording and review with GP as per guidelines.
• Completing assessments and personalised care plans: assess resident needs, monitor mobility, intake and safety; update care plans regularly.
• Leading a care team: on‑duty nurse coordinates staff, mentors others and ensures care standards are followed according to policy.
• Coordinating multi-disciplinary teams: liaise with GP, health professionals and referrals to ensure integrated resident care.
• Communication and record-keeping: maintain clear documentation, handovers, and update monitoring forms and risk assessments.
