City Health Care Partnership CIC is seeking a motivated Registered Nurse to join our Out of Hours Nursing Team, delivering high-quality unplanned care across Hull and East Riding. You will assess, plan and provide nursing care in patients’ homes and community settings, meeting urgent healthcare needs outside normal hours.
You’ll triage, assess and deliver safe, patient-centred care, maintain records and work with multidisciplinary teams to coordinate referrals.
