Job summary

Active Care Group is seeking an experienced Complex Care Support Worker to join as a Care Response Specialist in a regional role across North Wales. You’ll support planning, stabilisation and delivery of complex care packages, helping hospital discharges and ensuring high-quality, person-centred care in clients' homes.

The role requires a full UK driving licence, experience with complex needs and the ability to travel regionally.

Qualifications

Full UK Driving Licence
Experience supporting people with complex care needs
Confidence working independently
Excellent communication and organisational skills
A calm, compassionate and professional approach
Flexibility to travel regionally
Hospital discharge experience (desirable)

Responsibilities

Support the set-up, stabilisation and delivery of complex care packages
Assist with safe and timely hospital discharges
Deliver high-quality, person-centred care
Support clients with daily living activities and promote independence
Build routines that improve confidence and wellbeing
Provide reassurance and continuity for clients and families
Mentor and support new Support Workers
Collaborate with Care Managers and Clinical Professionals
Ensure medication, equipment and care supplies are in place and escalate concerns
