The role requires a Full UK Driving Licence, independent working style and excellent communication. You’ll travel regionally, with a £29,000 salary plus £3,000 car allowance and mileage reimbursement, reflecting the demanding but rewarding
Qualifications
- Full UK Driving Licence required.
- Experience supporting people with complex care needs.
- Ability to work independently with strong communication and organisational skills.
- Calm, compassionate and professional approach with flexibility to travel regionally.
Responsibilities
- Support safe and timely hospital discharges.
- Deliver high-quality, person-centred care.
- Assist clients with daily living activities and promote independence.
- Build routines that improve confidence and wellbeing.
- Provide reassurance and continuity for clients and families.
- Mentor and support new Support Workers.
- Work alongside Care Managers and Clinical Professionals.
- Ensure medication, equipment and care supplies are in place.
- Identify concerns and escalate appropriately.
