Job summary

Active Care Group in Newcastle upon Tyne is seeking a Care Response Specialist to deliver urgent, person-centred complex care across the region. You’ll help with safe hospital discharges, stabilise care packages and provide reassurance to families, all while working independently within a highly skilled team.

This role requires a full UK driving licence, experience with complex care needs, and the ability to travel regionally.

Responsibilities

• Support safe and timely hospital discharges.
• Deliver high-quality, person-centred care.
• Assist with daily living activities and promote independence.
• Build routines to improve confidence and wellbeing.
• Provide reassurance to clients and families and continuity of care.
• Mentor and support new Support Workers.
• Collaborate with Care Managers and clinical professionals.
• Ensure medication, equipment and supplies are in place.
• Identify concerns and escalate appropriately.
