Kathryn Rose Consultancy is recruiting two Complex Care Managers to lead high-quality community-based care packages across the West Midlands and North West. The roles require regional travel to client sites and close collaboration with clinical teams and local authorities to ensure safe, person-centered care for both adult and paediatric users.

Qualifications

• 5 years' experience in community or domiciliary care.
• 1+ year of clinical exposure to complex needs (e.g., tracheostomies, ventilation, SCI/ABI).
• Experience supporting both paediatric and adult service users.

Responsibilities

• Design and coordinate bespoke complex care plans.
• Maintain and audit care packages for safety and regulatory adherence.
• Conduct regular home visits within region to support service users and families.
• Collaborate with clinicians, ICBs, and local authorities to refine interventions.
• Draft and sign off comprehensive care plans aligned with standards.
• Prepare service reports for commissioners and participate in external reviews.
• Provide leadership and supervision to regional care teams.
