Job summary

Active Care Group seeks a Clinical Manager in Newcastle to lead clinical care across a regional caseload. You will ensure high-quality, personalised care in clients’ homes, coordinating with Care Managers, Co-ordinators and multidisciplinary teams.

You will hold RN/NMC registration and drive clinical governance, with a focus on safety, quality and regulatory compliance to support safe transitions from hospital to home.

Qualifications

• Registered Nurse (RN) or equivalent healthcare professional with current NMC registration – essential.
• Advanced qualifications in clinical governance, quality assurance, or risk management – desirable.
• Experience in a clinical leadership role, preferably within complex care, domiciliary care, or community healthcare settings.

Responsibilities

• Clinical management of an agreed caseload with clear care plans, tasks and competency requirements.
• Maintain skills and training to support client needs, including occasional out-of-hours support.
• New client assessments and mobilisation of complex care packages.
• Attend MDT meetings with internal and external stakeholders to support clinical care.
• Provide leadership, identify ideas, and drive flexible, responsive leadership.
• Liaise with funders and attend case conferences/MDT meetings as required.
