Job summary

BDS Recruitment is currently recruiting an organised and experienced Referral Coordinator to join a supported housing service from either Tolvaddon or St Austell office.

You will coordinate referrals, liaise with professionals and gather information to assess needs and risks, with a focus on safeguarding and working with domestic abuse clients.

Qualifications

• Experience in referrals, supported housing, homelessness, domestic abuse, housing, social care, safeguarding or community services.
• Previous experience in a coordination, referral, housing or support-based role.
• An understanding of risk assessment and safeguarding.

Responsibilities

• Coordinate and manage referrals into the service.
• Liaise with referring agencies, professionals and partner organisations.
• Gather and assess referral information to identify support needs and risks.
• Complete initial risk assessments and ensure relevant information is shared appropriately.
• Coordinate placements and liaise with the team regarding accommodation availability.
• Maintain accurate referral records, case notes and databases.

The main focus of this role is working with people fleeing domestic abuse who may have complex support needs.

You will be responsible for coordinating referrals into the service, working closely with professionals, partner agencies and the wider support team to ensure people are assessed and supported appropriately.

Main duties will include but not limited too;

• - Coordinate and manage referrals into the service.
• - Liaise with referring agencies, professionals and partner organisations.
• - Gather and assess referral information to identify support needs and risks.
• - Complete initial risk assessments and ensure relevant information is shared appropriately.

About you

• - You will ideally have experience in referrals, supported housing, homelessness, domestic abuse, housing, social care, safeguarding or community services.
• - Previous experience in a coordination, referral, housing or support-based role.
• - An understanding of risk assessment and safeguarding.

Location; Able to work from either Tolvaddon or St Austell office with home working also

Hours; Monday- Friday 9am- 5pm

Pay Rate; £15.34ph PAYE- £19.90ph UMB

This is a temp ongoing role to start ASAP!

You MUST hold an enhanced DBS covering Child and Adult Workforce on the update service or issued within the past 12 months.
