Job summary

Greater Manchester Mental Health NHS Foundation Trust is seeking a Team Administrator to support Referral and Triage Practitioners within the Specialist Community Forensic Team. You will coordinate patient referrals, ensure timely processing, and arrange assessments in line with local guidelines.

The role covers general office duties, minute taking, typing letters, managing mailboxes and answering calls from patients, carers and health professionals.

Responsibilities

• Coordinate patient referrals and direct written and telephone enquiries for referrals to the Specialist Community Forensic Team
• Handle patient and carer enquiries about the referral process with strict confidentiality
• Process referrals and ensure updates to clinical information systems align with Trust guidelines
• Monitor shared mailboxes and act promptly
• Liaise with external agencies, other hospitals, medical services and relatives ensuring clear information flow
• Record activity onto appropriate systems and meet deadlines
• Track service users from referral to discharge ensuring appointments are within targets
• Maintain accurate patient records including referral information
• Input high-quality data into the EPR to support quality improvement and performance
