The Forward Trust in the United Kingdom is seeking a dedicated Recovery Worker to deliver person-centred drug and alcohol support. You will work with service users to engage with psychosocial interventions, treatment programmes and joined-up care with healthcare partners.

The role requires flexibility, good communication, and willingness to travel between projects within the cluster to ensure continuity of care and positive outcomes.

Responsabilidades

• Delivering person-centred drug and alcohol treatment and recovery interventions tailored to each client's individual needs.
• Supporting service users to engage with psychosocial interventions and structured treatment programmes.
• Working closely with healthcare delivery partners to support all aspects of the Stoke Heath Integrated Care (SHIC) service at HMP Stoke Heath.
• Building effective working relationships with partner agencies to promote joined-up care and continuity of support.
• Contributing to service targets set by Forward and local commissioners.
• Monitoring and supporting client progress, helping to measure outcomes and demonstrate the impact of the service.
• Promoting recovery, wellbeing and positive change through consistent, client-focused support.
• Working flexibly to meet service needs, including occasional evening and weekend shifts.
• Providing support across other projects within the cluster when required, ensuring continuity of care for service users.
