Job summary

The Forward Trust in Thurrock is seeking a Recovery Coordinator to support adults with drug and alcohol challenges. You will manage a caseload, conduct assessments, develop risk and SMART care plans, and deliver person-centred psychosocial interventions to aid recovery.

Using harm reduction and abstinence-based approaches, you will run individual sessions, group-work, and collaborate with housing, health, probation and other services to ensure continuity of care and positive outcomes.

Qualifications

• Experience of working within substance misuse services.
• Ability to deliver structured psychosocial interventions and facilitate both individual and group sessions.
• Experience developing risk management plans and implementing SMART care plans.
• Understanding of safeguarding and equality, diversity and inclusion.

Responsibilities

• Deliver structured one-to-one interventions and facilitate group-work sessions.
• Support structured treatment programmes and create SMART care plans.
• Build effective partnerships with housing providers, employment services, healthcare professionals, probation and other agencies to ensure continuity of care.
• Maintain accurate records, participate in clinical supervision and contribute to service targets.
