Job summary

Forward Trust is seeking a dedicated Recovery Navigator to support individuals on probation across London boroughs. Based in various London boroughs, the role involves face-to-face client work, flexible travel between delivery units, and delivering tailored recovery-focused interventions.

You will work within a multidisciplinary team to assess needs, create action plans, and provide advocacy and motivational support to help clients access services and sustain positive change.

Qualifications

• Experience supporting individuals with substance misuse or complex needs.
• Strong communication and partnership-building skills.
• Flexible, proactive approach and commitment to empowering others.
• Ability to work across multiple sites within London.

Responsibilities

• Manage a diverse caseload of adult male prison leavers and those on community orders.
• Complete comprehensive assessments and create collaborative action plans.
• Deliver one-to-one and group interventions (including virtual sessions).
• Maintain accurate and timely records.
• Offer practical support and advocacy, from completing referral forms and attending appointments to liaising with partner agencies.
• Provide advice, guidance and information that empowers individuals to act and progress.
• Deliver motivational interventions to build recovery skills and confidence.
