Job summary

Somerset NHS Foundation Trust's Mental Health and Learning Disabilities Directorate is seeking a Support Time and Recovery Worker to deliver evidence-based interventions to individuals with severe mental health conditions in Community Mental Health Service.

You will work in the Assertive Outreach Team, supporting in-community care, facilitating discharge from inpatient to home, and liaising with NHS, VCSE partners, housing and social care to promote recovery.

Responsibilities

• Deliver recovery-focused and evidence-based interventions to individuals with severe and enduring mental health difficulties.
• Support service users to work towards goals identified within their care plans and promote engagement with treatment and recovery.
• Provide practical and emotional support to individuals within their homes and community settings.
• Support patients during transitions between inpatient and community services, helping to facilitate successful discharge and recovery.
• Develop therapeutic and professional relationships with service users, carers and families.
• Promote independence, wellbeing and social inclusion through person-centred support.
• Identify early signs of relapse or deterioration and elevate concerns appropriately to qualified team members.
• Work collaboratively with a wide range of partner agencies including healthcare providers, housing services, voluntary organisations and social care teams.
• Support service users to access services relating to accommodation, benefits, advocacy and community resources where appropriate.
• Maintain accurate records and contribute to service development, quality improvement and evaluation activities.
• Participate in supervision, team meetings and ongoing training and development opportunities.
