Job summary

Change Grow Live Norfolk is recruiting a Recovery Coordinator to join the Intensive Support Team in Nottinghamshire. The post is full-time (37.5 hours per week) on a fixed-term contract to 31 March 2028, offering rewarding work helping people with substance and alcohol issues access holistic support.

• Experience of substance misuse and mental health support.
• Ability to work with multiple agencies and services.
• Strong communication, advocacy and recording skills.

Responsibilities

• Manage a caseload of service users via Intensive Support Panel.
• Develop wrap-around plans with partners and housing teams.
• Work with mental health agencies to address issues.
• Maintain accurate records and ensure safeguarding Procedures.
• Participate in multi-agency meetings and case conferences.
• Provide person-centered guidance and support to service users.
