Job summary

Target Housing in Sheffield is seeking an experienced Recovery Case Worker to join the St Bart's team. The role involves empathetic, person-centred support for service users with complex needs, including mental health and learning disabilities, delivered both 1:1 and in groups.

You will work with the Service Manager and Recovery Team Leader to manage a caseload, develop tenancy sustainment plans, and coordinate risk management and independent living supports.

Qualifications

• A relevant qualification in mental health, learning disabilities, health and social care, or similar.
• Experience of supporting people with multiple complex needs, particularly with mental health and learning disabilities.
• Experience of building successful professional relationships both with service users, colleagues and professional networks.
• Knowledge and understanding of PIE (Psychologically Informed Environments) and TIP (Trauma Informed Practice).
• Strong organisational and communication skills.

Responsibilities

• Support the delivery of high quality 1:1 and group activities in people’s homes and in the community.
• Manage and advocate for a caseload of service users who have needs around mental health; learning disabilities; multiple disadvantages.
• Develop and maintain effective working relationships with relevant agencies to provide a joined-up network of support for clients.
• Keep and update accurate records on the case management system.
