Job summary

Target Housing in Sheffield is seeking a Recovery Case Worker to join St Bart’s team, delivering empathetic, person-centred support to service users in community and home settings. You will manage a caseload focused on tenancy sustainment, risk and planning, collaborating with partner agencies for a joined-up network of care.

The role requires experience with individuals who have multiple complex needs, including mental health and learning disabilities, and a commitment to helping people move

Qualifications

• A relevant qualification in mental health, learning disabilities, health and social care, or similar.
• Experience of supporting people with multiple complex needs, particularly with mental health and learning disabilities.
• Experience of building successful professional relationships with service users, colleagues and networks.
• Knowledge of PIE and TIP.
• Strong organisational and communication skills.

Responsibilities

• Support the delivery of high quality 1:1 and group activities in people\'s homes and in the community.
• Manage and advocate for a caseload of service users with mental health needs and learning disabilities.
• Develop and maintain effective working relationships with relevant agencies for joined-up support.
• Keep and update records on the case management system.

1:1 Recovery Case Worker - St Barts

Location: S6 3QA

Salary: £26,490 per annum

Shift pattern: 12-hour shifts over a 24/7 rota - nights only

Contract Type: Permanent, Full-Time

We're looking for an experienced and compassionate Recovery Case Worker to join our dedicated team in Sheffield.

Working closely with our Service Manager and Recovery Team Leader, you will play a key role in supporting our service at St Bart's. As a proactive Recovery Case Worker, you will have an empathetic and person-centred approach, ensuring service users receive the dedicated support they require.

The successful candidate will have experience working with individuals with multiple and complex needs, and will be confident in delivering support both 1:1 and in group settings. You will have a strong understanding of tenancy sustainment, risk management, and support planning. Above all, we are looking for someone who is empathetic, organised, and committed to helping people achieve their potential and move towards independent living.

General Tasks

• Supporting the delivery of high quality 1:1 and groups activities in people's homes and in the community
• Manage and advocate for a caseload of service users who have needs around mental health; learning disabilities; multiple disadvantages
• Develop and maintain effective working relationships with relevant agencies to provide a joined-up network of support for clients
• Keep and update accurate records on the case management system

Requirements
