Job summary

Target Housing is seeking an experienced Recovery Case Worker for our Sheffield-based service at St Bart's. You will deliver empathetic, person-centred support, working with individuals with complex needs to promote tenancy sustainment and independent living.

You will liaise with the Service Manager and Recovery Team Leader, providing proactive case management, risk assessment, and planning while delivering both 1:1 and group support to clients and their networks.

Qualifications

• Strong organisational and communication skills are essential.
• Experience supporting people with multiple complex needs is required.
• Knowledge of PIE and TIP approaches is beneficial.

Responsibilities

• Support high quality 1:1 and group activities in homes and the community.
• Manage a caseload of service users with mental health needs and learning disabilities.
• Develop and maintain relationships with agencies to provide coordinated support.
• Keep accurate records on the case management system.
