Job summary

Waythrough is seeking an Aftercare Practitioner at Peterlee Health Centre to support individuals in recovery from drug and alcohol use. You will manage a personal caseload, develop recovery plans, and deliver one-to-one and group sessions, in a multidisciplinary team setting.

You will collaborate with families and partner organisations to provide joined-up support, maintaining a person-centred approach, with emphasis on emotional wellbeing, safeguarding and risk management.

Qualifications

• NVQ Level 3 in Health and Social Care or equivalent qualification, or willingness to work towards it.
• Excellent communication skills with compassionate, professional boundaries.
• Understanding of recovery-focused support and evidence-based interventions.
• Experience with families and carers; record-keeping competencies are vital.

Responsibilities

• Build and maintain case management records and plans.
• Deliver one-to-one and group recovery interventions.
• Develop and review personalised recovery plans and risk assessments.
• Collaborate with families, carers and partner organisations for joined-up support.
• Promote safeguarding and harm reduction in all activities.
• Work within a multidisciplinary team to support individual goals.
• Provide written reports and contribute to service monitoring and outcomes evaluation.
• Contribute to service development and an inclusive environment.
