Job summary

Waythrough is seeking an Aftercare Practitioner to help people in recovery from drug and alcohol use at Peterlee Health Centre. You will work within a multidisciplinary team, managing your caseload, developing recovery plans and delivering interventions face-to-face and online.

You will provide person-centred, holistic support, build resilience, engage families where appropriate, and ensure safeguarding and accurate case records. This is a full-time, permanent role at 37 hours per week.

Qualifications

• NVQ Level 3 in Health and Social Care or willingness to work towards this.
• Excellent communication skills and compassionate engagement.
• Understanding of recovery-focused support and evidence-based interventions.
• Experience with families and carers.
• Ability to maintain accurate records using IT.

Responsibilities

• Build trusting relationships to support ongoing recovery and personal goals.
• Complete assessments, develop and review recovery plans, and undertake risk assessments.
• Deliver recovery groups and one-to-one sessions using evidence-based approaches.
• Engage families and carers to support home and community stability.
• Collaborate with partner agencies to improve access to services.
• Provide harm reduction advice and follow safeguarding procedures.
• Maintain up-to-date case records and contribute to monitoring and reporting.
• Share knowledge to help develop the service and promote an inclusive environment.
