Job summary

Lancashire Care Foundation Trust is recruiting a Reconnect Practitioner to support people in East Lancashire and across Lancashire as needed. The role forms part of ReConnect Care after Custody for Lancashire & Cumbria, assisting prisoners up to 12 weeks before release and up to 6 months post-release.

The post holder will work to screen and assess vulnerabilities, coordinate links to community health services, and help individuals transition safely from prison to community-based care and support.

Qualifications

• Screen and assess people approaching release for vulnerabilities.
• Provide support within the community after release.

Responsibilities

• Identify and respond to requests to screen and assess vulnerable individuals.
• Provide ongoing support to ensure engagement with community services.
