Job summary

The Reconnect Care after Custody service, run by Lancashire & South Cumbria NHS Foundation Trust, supports people leaving six prisons. The practitioner triages, assesses and refers them from up to three months before release, with support for up to six months after.

Work is with people facing complex needs such as homelessness, substance misuse and mental health, with hybrid working across Trust sites and home. Applicants must hold the right to work in the UK.

Qualifications

• Diploma in Health and/or Social Care or equivalent experience.
• Level 3 NVQ in a health-related subject.
• Experience with adults with vulnerabilities and complex needs.

Responsibilities

• Engage with people with vulnerabilities and complex needs.
• Assess and support access to services for identified needs.
• Work with people in prison before release and provide follow-up after release.
• Help move from prison into community-based healthcare and support.
