Job summary

Qualifications

• Experience engaging with adults with vulnerabilities including homelessness, substance misuse, mental health and complex needs.
• Strong verbal and written communication skills.
• Ability to work across prison and community settings with vetting requirements.

Responsibilities

• Actively engage individuals with complex needs and support access to essential health and care services.
• Provide a bridge between prison and community healthcare, ensuring smooth transitions post-release.
