Job summary

Leaf Complex Care, part of the Catalyst Care Group, is seeking a Healthcare Assistant to provide compassionate, person-centered care primarily in clients' homes, helping people live independently with dignity. You will support daily living activities, assist with personal care, plan community activities, and maintain clear communication with families and care teams.

Eligibility includes NVQ Level 3 preferred (Level 2 acceptable) and at least 6 months in similar healthcare settings.

Qualifications

  • NVQ Level 3 in Health and Social Care is preferred, Level 2 acceptable.
  • Minimum 6 months of experience in similar healthcare settings, with complex needs, behaviours that challenge and autism.
  • Business Car Insurance would be advantageous.
  • Full UK driving license would be preferred.

As a Healthcare Assistant, you will provide tailored compassionate care primarily in the homes of individuals and supporting them to live independently and with dignity within their communities.

Responsibilities:
  • Deliver high-quality, person-centered care to individuals’ homes and within their communities.
  • Support individuals in maintaining or developing new skills and personal interests.
  • Adapt care and support plans to meet the diverse needs of each individual, ensuring their comfort and safety.
  • Engage in proactive communication with families and care teams to ensure effective care delivery.
  • Support individuals in temporary residential placements or their own homes.
  • Preform household chores such as laundry, cleaning and food shopping.
  • Manage and support individuals with budgeting and financial management.
  • Assists with all aspects of personal care, indulging hygiene, dressing and feeding.
  • Support individuals in maintaining or developing new skills and personal interests.
  • Plan, participate in and budget for community activities.
  • Maintain accurate records and clear communication with the brand team and other healthcare professionals.
Qualifications and Experience:
