As a Healthcare Assistant, you will provide tailored compassionate care primarily in the homes of individuals and supporting them to live independently and with dignity within their communities.

Responsibilities:

• -Deliver high-quality, person-centered care to individuals’ homes and within their communities.
• -Support individuals in maintaining or developing new skills and personal interests.
• -Adapt care and support plans to meet the diverse needs of each individual, ensuring their comfort and safety.
• -Engage in proactive communication with families and care teams to ensure effective care delivery.
• -Support individuals in temporary residential placements or their own homes.
• -Preform household chores such as laundry, cleaning and food shopping.
• -Manage and support individuals with budgeting and financial management.
• -Assists with all aspects of personal care, indulging hygiene, dressing and feeding.
• -Support individuals in maintaining or developing new skills and personal interests.
• -Plan, participate in and budget for community activities.
• -Maintain accurate records and clear communication with the brand team and other healthcare professionals.
