Job summary

Leaf Complex Care, part of Catalyst Care Group, is seeking a Healthcare Assistant to provide compassionate, home-based care that supports independence and dignity within the community. You will contribute to tailored care plans and work closely with families and care teams to ensure continuity and quality of care.

The role offers flexible working hours, opportunities for specialist training, and on-shift support through an on-call system.

Responsibilities

• Deliver high-quality, person-centered care to individuals in their homes and communities.
• Support individuals in maintaining or developing skills and personal interests.
• Adapt care plans to meet diverse needs, ensuring comfort and safety.
• Engage in proactive communication with families and care teams.
• Support individuals in temporary residential placements or their own homes.
• Perform household tasks such as laundry, cleaning and food shopping.
• Manage budgeting and financial tasks for individuals.
• Assist with personal care including hygiene, dressing and feeding.
• Plan, participate in and budget for community activities.
• Maintain accurate records and communicate clearly with the brand team and healthcare professionals.

• -Plan, participate in and budget for community activities.
• -Maintain accurate records and clear communication with the brand team and other healthcare professionals.
