Job summary

Honeycomb Group is seeking a Home Support Worker in Derbyshire to provide housing-related support to vulnerable and older people in their own homes. You will build trusted relationships, identify needs and support customers to achieve meaningful outcomes with practical guidance.

You will deliver person-centred, low-level support, carry out holistic assessments, co-produce support plans using Outcome Star, and help customers stay independent while signposting to local services.
