Job summary

Age Cymru Dyfed is recruiting a Mental Health Service Project Support Officer in Aberystwyth. This part-time, contract role operates in a hybrid pattern, with three days remote work and on-site use as needed. Salary starts from £23,296 per year, pro rata.

You will liaise with clients, provide telephone support, maintain accurate records, assist with referrals and research, and support mid-point assessments to ensure continued service value.

• Liaising with clients to complete initial introductions to the service and manage expectations
• Provide telephone support to clients who would like an alternative to face-to-face support and carry out regular well-being checks for those who choose to delay service.
• Ensure that accurate client records are kept for each element of service provision made through the service.
• Support engagement officers with onward referrals and research where further ‘added value’ support could be offered either through Age Cymru Dyfed’s other services or another service provider.
• Support mid-point assessments – review current support provisions and report any concerns to Engagement Officers
