Job summary

Responsibilities

• Support planned care for older adults, adults with physical disabilities and dementia under Social Services and Well-being Act.
• Assist in care planning and coordinating services with carers and partner agencies.
• Collaborate with multidisciplinary teams to ensure safe, person-centred support.

We have an opportunity in Ceredigion to recruit into a Social Work Assistant post to work in the Planned Care Team with older adults, adults with physical disabilities and psychiatry of old age who require care and support under Part 3 and 4 of the Social Services and Well-being Act and with carers who require support.

The Authority is committed to a strengths based approach using Signs of Safety and is in the process of transformation to a through-age and whole family model which offers greater flexibility to develop plans & services to meet needs. This model provides exciting opportunities for collaborative working between departments within the authority as well as with our partner agencies.

We are seeking Individuals with a degree of fluency in the Welsh language, A successful applicant without the required Welsh language skills will be fully supported to reach the desired standard within 2 years of appointment.
