Job summary

Liquid Personnel is seeking a Team Manager to join its client’s Reablement Team in Cheshire. You will lead a social work team, oversee case allocation, and ensure timely assessments, reviews, and discharge planning in line with statutory duties.

You will work collaboratively with health and community partners to troubleshoot barriers and deliver seamless, outcome-focused reablement support. This role begins on a 3-month contract with an immediate start.

Qualifications

• Registration with Social Work England is required.
• Eligible to work in the UK.
• Hold a full UK driving licence.
• Post-qualified experience is desirable.
• Degree level or equivalent in Social Work is required.

Responsibilities

• Lead and supervise the reablement social work team, providing guidance and reflective supervision.
• Coordinate and manage workflow, including allocation of cases and timely assessments, reviews, and discharge planning.
• Collaborate with health and community partners to ensure seamless, outcome-focused reablement support.
