Job summary

Leicestershire County Council's HART service seeks a Reablement Team Leader (Charnwood locality) to lead the Reablement Team in the community and work with health colleagues within a hospital setting to support timely discharges.

You will oversee first visits, risk assessments, and day-to-day line management of Reablement Support Workers, ensuring care plans help service users regain independence and achieve meaningful activities.

Qualifications

• Relevant experience working in adult social care settings.
• Have an NVQ 3 or equivalent and relevant experience working with adults in a social care setting is essential.
• Ability to use Information Technology applications (e.g. Outlook, Word, Excel and database applications).
• Ability to prioritise and manage own workload and meet deadlines.
• Ability to communicate clearly and accurately, both orally and in writing, with a wide range of people.
• Working on own initiative and as part of a team and liaising with other agencies to deliver integrated services.
• Ability and willingness to travel and work from various locations throughout the County; access to a vehicle.

Responsibilities

• Undertake first visits and risk assessments.
• Provide day-to-day line management support to Reablement Support Workers.
• Undertake Care Act Assessments where ongoing eligible need is identified.
• Collaborate with Occupational Therapists and Physiotherapists to deliver Reablement plans.
• Help service users identify and achieve goals related to daily living tasks.
