Job summary

Darlington Borough Council is seeking caring, motivated individuals to join our Reablement Team as Support Workers. This part‑time, permanent role offers the chance to make a real difference by helping adults regain independence at home and in their local communities after illness, injury, or hospital discharge.

Responsibilities

• Support individuals to achieve goals identified within their reablement plan.
• Assist people to develop daily living skills including personal care and meal preparation.
• Encourage independence, dignity and wellbeing.
• Support service users following hospital discharge or periods of ill health.
• Work collaboratively with Occupational Therapists, Social Workers and family members.
• Maintain accurate records and provide progress feedback.
• Follow safeguarding procedures and health and safety requirements.
• Build positive relationships with service users and carers.
